Ill bet you can do it yourself and have it turn out well enough to brag about. Or get the wife or a neighborhood kid to suit up with rubber boots. string lines to insure a slightly sloped surface for runoff. I string line level ($2) is priceless. The key is good forms, expansion joints, and inner reinforcment. If its going to be subject to vehicle traffic, use a 2x4 or 4x4 steel mesh. Pour 4-6 inches with medium aggregate.
If you call the local ready mix and give them dimesions, they will bring enough to get the job done and know the mix you need. Ive done a million small projects and often enough have asked the ready mix folks not to go out of their way for me, just stop by with the remnants from a larger job and this usually keeps the cost down. I have not meet a driver who will not help you pour, rake and screed to a point where typically all you need to do is float/trowel it off, cut your edges, and after it sets up, hit it with a broom and a bit of light spray with the hose for a textured surface (most common with driveways...not slippery, and doesnt show tiretracks and stains as well) good luck.
